Seeing a map tile, gaining a location marker, and actually opening a location are three separate processes. Entering a world-map tile reveals the current tile and by default checks a 3×3 square for markable locations; it does not reveal every surrounding tile on every step.

Three separate processes
The table separates fog reveal, gaining a lead, and opening a location in their actual execution order. A location marker alone does not prove that the player has entered it.

How to plan exploration
While exploring on foot, read 3×3 as the nearby location-marker check. Usually the current tile is the direct fog reveal; adjacent tiles meeting a dedicated condition and special terrain may also reveal, so the visible edge can sometimes expand farther.
For example, if the current tile is (10,10), (11,11) is inside the ordinary location-check area, while (12,10) is outside it. Being in range is not enough: the tile must still be fogged, have an eligible location type, and lack an existing rumor marker; near a map edge, only valid map tiles can meet those conditions.
Dialogue and paper maps use different defaults: a 5×5 square for location checks and an actual 3×3 paper-map reveal. Reconnaissance is an exception that checks a 13×13 square around the player for locations without revealing paper-map tiles through that call.
Dialogue rumors and leads generated by approaching a location use the same map rumor marker. Reaching and opening the location for the first time clears that rumor marker and records it as open; the character's current XP modifiers still apply before the reward is awarded.
The site's interactive map shows example placements under the same-release rules. Use it to recognise possible wilderness-event types, rather than treating example coordinates as fixed across every save.
